Schizophrenia Pharmacology: Past, Present, and Future Targets for Intervention (Will J. Cronenwett, July 13, 2016)

FOCUS, Vol 14. Issue 3. Summer 2016, pp. 308-314

http://dx.doi.org/10.1176/appi.focus.20160009

The first medication for schizophrenia was discovered serendipitously. Years later, it was shown that the medication worked by blocking dopamine, and to this date, all available antipsychotic medications also work by blocking dopamine. They differ, however, in many other respects.

The so-called first-generation medications have a wide range of receptor affinities, but in all cases, they have a higher affinity for dopamine receptors than for serotonin receptors.

In contrast, so-called second-generation medications have a higher affinity for serotonin receptors than for dopamine receptors.
A third category of medication acts as a partial agonist at the dopamine receptor
It is likely that a fourth category will also become available and these medications will act as agonists at the N-methyl-d-aspartate receptor, although clinical trials thus far have struggled to demonstrate efficacy.
In addition to medications that treat symptoms of psychosis, medications are under development to directly target some of the more fundamental aspects of cognition that are impaired in schizophrenia, including memory, sensory processing, attention, and executive function. Several promising strategies are discussed.

Beyond Psychopharmacology: Emerging Psychosocial Interventions for Core Symptoms of Schizophrenia (Peter J. Weiden, July 13, 2016)

FOCUS: The Journal of Lifelong Learning in Psychiatry, Vol 14, Issue 3, Summer 2016, pp. 315-327

http://focus.psychiatryonline.org/doi/10.1176/appi.focus.20160014

Psychiatrists who work with patients with severe mental illness often are more focused on diagnosis, medical management, and psychopharmacology than on psychosocial treatments. Furthermore, many psychosocial interventions that show great promise may not be available locally, making it harder for psychiatrists to recognize emerging trends. Finally, there has not been an update in the American Psychiatric Association’s Practice Guideline for the Treatment of Schizophrenia for many years, and the most recent Patient Outcomes Research Team (PORT) review of evidence-based psychosocial treatments for schizophrenia was published eight years ago.
This article reviews a selection of psychosocial interventions that have shown success in treating some of the more vexing and persistent core schizophrenia symptoms that often continue despite optimal pharmacologic treatment; formerly these had been considered too risky or out of reach for psychosocial intervention.
The interventions reviewed include cognitive-behavioral therapy for psychosis (CBTp), which aims to reduce distress and disability from psychotic symptoms; CBT and other behavioral interventions focused for comorbid posttraumatic stress syndrome; cognitive training (remediation) interventions that use computerized training programs to reduce the severity and consequence of cognitive impairment associated with schizophrenia; clubhouse and peer support models that address the social alienation and social defeat endemic to persons with severe mental illness; and supported employment interventions that are effective in helping patients get back to work in a competitive job environment.
The interventions are reviewed with the needs of the prescribing mental health clinician in mind. Each intervention’s strengths and weaknesses are described, as well as their role in recovery-oriented treatment services.

Boundaries and Boundary Violations in Psychoanalysis 2 ed (Glen Gabbard, March 1, 2016)

The first edition of Boundaries and Boundary Violations in Psychoanalysis, published 20 years ago, was groundbreaking in addressing a subject that was at that time considered taboo. This new, thoroughly revised edition contains a wealth of new material, reflecting the current knowledge and evolving attitudes about boundary violations. The volume, which is based on the author's 30 years of experience evaluating, treating, and consulting on more than 300 cases of professional boundary violations, is essential reading for psychoanalysts, residents, training directors, therapists, and other mental health professionals. Perhaps the most obvious form of boundary violation is sexual, and the author examines how these situations develop, how they destroy the analytic relationship, and how different types of analysts transgress boundaries for different motives, both unconscious and conscious. Other forms of boundary violations (for example, nonsexual and supervisory), are also addressed, including a fascinating and instructive chapter on cyber violations, a type of boundary violation that could not have been predicted 20 years ago. Beautifully written and meticulously referenced, Boundaries and Boundary Violations in Psychoanalysis is an exhaustive, utterly current examination of a critically important topic and represents a vital contribution to the psychoanalytic literature.

but I must go on the frontiers are my prison

When they poured across the border
I was cautioned to surrender,
This I could not do;
I took my gun and vanished.

I have changed my name so often,
I've lost my wife and children
But I have many friends,
And some of them are with me.

An old woman gave us shelter,
Kept us hidden in the garret,
Then the soldiers came;
She died without a whisper.

There were three of us this morning
I'm the only one this evening
But I must go on;
The frontiers are my prison.

Oh, the wind, the wind is blowing,
Through the graves the wind is blowing,
Freedom soon will come;
Then we'll come from the shadows.

Les allemands e'taient chez moi,
Ils me dirent, "r©signe toi,"
Mais je n'ai pas peur;
J'ai repris mon ¢me.

The germans were at my home
They said, "sign yourself,"
But I am not afraid
I have retaken my soul.

J'ai change' cent fois de nom,
J'ai perdu femme et enfants
Mais j'ai tant d'amis;
J'ai la france entie're.

I've changed names a hundred times
I have lost wife and children
But I have so many friends
I have all of france

Un vieil homme dans un grenier
Pour la nuit nous a cache',
Les allemands l'ont pris;
Il est mort sans surprise.

An old man, in an attic
Hid us for the night
The Germans captured him
He died without surprise.

Oh, the wind, the wind is blowing,
Through the graves the wind is blowing,
Freedom soon will come;
Then we'll come from the shadows.

Brothers In Arms (Mark Knopfler, Berlin 2007)

These mist covered mountains
Are a home now for me
But my home is the lowlands
And always will be
Someday you'll return to
Your valleys and your farms
And you'll no longer burn
To be brothers in arms

Through these fields of destruction
Baptisms of fire
I've witnessed your suffering
As the battle raged higher
And though they did hurt me so bad
In the fear and alarm
You did not desert me
My brothers in arms

There's so many different worlds
So many different suns
And we have just one world
But we live in different ones

Now the sun's gone to hell and
The moon's riding high
Let me bid you farewell
Every man has to die
But it's written in the starlight
And every line in your palm
We are fools to make war
On our brothers in arms

The Interpersonal Perspective in Psychoanalysis, 1960s-1990s: Rethinking transference and countertransference (2017)

The Interpersonal Perspective in Psychoanalysis, 1960s-1990s: Rethinking transference and countertransference, ed. by Donnel B. Stern, Irwin Hirsch, Routledge, March 3, 2017

North American psychoanalysis has long been deeply influenced and substantially changed by clinical and theoretical perspectives first introduced by interpersonal psychoanalysis. Yet even today, despite its origin in the 1930s, many otherwise well-read psychoanalysts and psychotherapists are not well informed about the field. The Interpersonal Perspective in Psychoanalysis, 1960s to 1990s provides a superb starting point for those who are not as familiar with interpersonal psychoanalysis as they might be. For those who already know the literature, the book will be useful in placing a selection of classic interpersonal articles and their writers in key historical context.
During the time span covered in this book, interpersonal psychoanalysis was most concerned with revising the understanding of the analytic relationship―transference and countertransference-and how to work with it. Most of the works collected here center on this theme. The interpersonal perspective introduced the view that the analyst is always and unavoidably a particular, "real" person, and that transference and countertransference need to be reconceptualized to take the analyst’s individual humanity into account. The relationship needs to be grasped as one taking place between two very particular people. Many of the papers are by writers well known in the broader psychoanalytic world, such as Bromberg, Greenberg, Levenson, and Mitchell. But also included are those by writers who, while not as widely recognized beyond the interpersonal literature, have been highly influential among interpersonalists, including Barnett, Schecter, Singer, and Wolstein.
Donnel B. Stern and Irwin Hirsch, prominent interpersonalists themselves, present each piece with a prologue that contextualizes the author and their work in the interpersonal literature. An introductory essay also reviews the history of interpersonal psychoanalysis, explaining why interpersonal thinking remains a coherent clinical and theoretical perspective in contemporary psychoanalysis. The Interpersonal Perspective in Psychoanalysis, 1960s to 1990s will appeal greatly to psychoanalysts and psychoanalytic psychotherapists wanting to know more about interpersonal theory and practice than can be learned from current sources. Lectures on Technique by Melanie Klein: Edited with critical review by John Steiner (2017)

Lectures on Technique by Melanie Klein: Edited with critical review by John Steiner, by Melanie Klein, ed. by John Steiner, Routledge, January 27, 2017

Melanie Klein’s Lectures on Technique is based on a series of six lectures given by Melanie Klein to students at the British Psychoanalytical Society in 1936 and repeated several times in subsequent years. They were discovered in the Melanie Klein Archives housed in the Wellcome Medical Library and have been previously described by Elisabeth Spillius but never before published. In this book, John Steiner explores what characterises Kleinian Technique, how her technique changed over the years, what she saw as the correct psychoanalytical attitude and also how psychoanalytic technique has changed since Klein’s death.
Melanie Klein, who moved to England from Berlin in 1927, became one of the leading psychoanalysts, following Freud and making an important contribution in the theory and practice of psychoanalysis. A pioneer in child analysis, her work remains widely influential throughout the world. This book consists of the full text of the original six lectures, accompanied by a critical analysis from John Steiner who is known internationally as a leading Kleinian analyst and writer. They demonstrate the importance of the lectures in understanding Klein’s work and its continued relevance for contemporary psychoanalysis. In addition, also published for the first time, this book includes annotated transcripts of a preserved recording of a seminar Klein held in 1958 with young analysts of the British Psychoanalytical Society. In this seminar, close to the end of her life, many of the points made in the earlier lectures were elaborated upon and brought further up to date in light of developments in Klein’s thinking during the intervening years.
Featuring rare, previously unpublished material, Melanie Klein’s Lectures on Technique provides a new and significant contribution to understanding of the Kleinian paradigm. It will be essential reading for all psychoanalysts and psychoanalytic psychotherapists interested in and influenced by Klein’s work and legacy. (amazon)
