Key Points
Question  Which medications provide the best delirium response rate, the lowest delirium occurrence rate, and the best tolerability for the treatment and prevention of delirium?
Findings  From the results of a network meta-analysis of 58 randomized clinical trials among 9603 individuals, haloperidol plus lorazepam had the best response rate for delirium treatment, and ramelteon had the lowest delirium occurrence rate. No pharmacological management was significantly associated with a higher risk of all-cause mortality compared with placebo or control groups during delirium treatment or prevention.
Meaning  The use of a combination of haloperidol plus lorazepam and ramelteon is suggested for the treatment and prevention of delirium.
Abstract
Importance  Although several pharmacological interventions for delirium have been investigated, their overall benefit and safety remain unclear.
Objective  To evaluate evidence regarding pharmacological interventions for delirium treatment and prevention.
Data Sources  PubMed, Embase, ProQuest, ScienceDirect, Cochrane Central, Web of Science, ClinicalKey, and ClinicalTrials.gov from inception to May 17, 2018.
Study Selection  Randomized clinical trials (RCTs) examining pharmacological interventions for delirium treatment and prevention.
Data Extraction and Synthesis  To extract data according to a predetermined list of interests, the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines were applied, and all meta-analytic procedures were conducted using a random-effects model.
Main Outcomes and Measures  The primary outcomes were treatment response in patients with delirium and the incidence of delirium in patients at risk of delirium.
Results  A total of 58 RCTs were included, in which 20 RCTs with 1435 participants (mean age, 63.5 years; 65.1% male) compared the outcomes of treatment and 38 RCTs with 8168 participants (mean age, 70.2 years; 53.4% male) examined the prevention of delirium. A network meta-analysis demonstrated that haloperidol plus lorazepam provided the best response rate for delirium treatment (odds ratio [OR], 28.13; 95% CI, 2.38-333.08) compared with placebo/control. For delirium prevention, the ramelteon, olanzapine, risperidone, and dexmedetomidine hydrochloride groups had significantly lower delirium occurrence rates than placebo/control (OR, 0.07; 95% CI, 0.01-0.66 for ramelteon; OR, 0.25; 95% CI, 0.09-0.69 for olanzapine; OR, 0.27; 95% CI, 0.07-0.99 for risperidone; and OR, 0.50; 95% CI, 0.31-0.80 for dexmedetomidine hydrochloride). None of the pharmacological treatments were significantly associated with a higher risk of all-cause mortality compared with placebo/control.

Conclusions and Relevance  This network meta-analysis demonstrated that haloperidol plus lorazepam might be the best treatment and ramelteon the best preventive medicine for delirium. None of the pharmacological interventions for treatment or prophylaxis increased the all-cause mortality.

The state of play: The country justifiably wants a bigger say now, but its increasing authoritarianism is raising concerns about its efforts to reshape global governance.

From the American perspective: Earlier this week, the Center for American Progress issued a report on China's global governance ambitions. Among the highlights...

The current global governance system is rules-based, and it privileges liberal democratic values and standards; Beijing’s alternative vision is a system based on authoritarian governance principles in which nations negotiate issues bilaterally instead of following common rules and standards.

From a liberal democratic perspective, if Beijing succeeds in bringing about that vision, the world will be less free, less prosperous, and less safe.

This report aims to provide a deeper and more nuanced perspective on China’s real global governance intentions by mapping what President Xi and other Chinese leaders are saying to their domestic audience and how Chinese foreign policy scholars interpret those statements...

Deeper analysis indicates that there is ample reason for concern. China’s stated goals include watering down liberal democratic principles and either replacing or augmenting them with authoritarian ones. At the same time, this analysis also reveals that liberal democracies have powerful levers for shaping China’s actions in the global governance space.

兩年前曾大爆香港和中國大陸官場內幕的《成報》,老闆谷卓恆近日在推特上發文,指控中國國家主席習近平家族貪腐嚴重,超過了以前的元老領導人江、朱、李、胡、溫、曾等家族。谷卓恆是商人出身,因涉嫌銀行貸款詐騙曾被警方通緝,也涉及網路P2P貸款平臺問題,外傳還具有中國情報人員背景,頗為神秘和複雜,他揚言接下來將持續爆料習近平家族的粵港資產。

根據香港《蘋果日報》、自由亞洲電台(RFA)及洛杉磯新聞中文網的報導,谷卓恆本人目前在美國,已經和美國聯邦調查局進行合作,但無法獲得證實。谷卓恆回應稱,自己受到了跟蹤和電話威脅,但絕不會妥協,也不會害怕威脅,並且「有信心把那把破鐮刀和那把破斧頭(意指中共)扔進歷史的垃圾堆」。

最近一年多保持低調的谷卓恆,突然在推特上發出指控習近平家族腐敗的帖文。谷卓恆爆料指出,從香港被抓回的大陸富商肖建華的資產,被一間名為秦川大地的公司接管,涉及資金規模有萬億美元。而秦川大地公司的股東正是習近平的姐夫鄧家貴,他所控制的資金規模,遠遠超過其他中共權貴家族。

由中國政府控制的香港《大公報》便曾公開指控他是經濟罪犯,並說他已在國際刑警紅色通緝令名單中。另有媒體報導稱,谷卓恆的另一個身分是中國總參情報二部的特工。

根據中國的互動百科,谷卓恆因涉嫌銀行貸款詐騙曾被警方通緝,他也涉及一些網路P2P貸款平臺問題。《成報》是香港歷史最長的報紙之一,後因經營困難,2014年,由谷卓恆入股,成為該報董事局主席。有報導說,《成報》目前的財務困境並未解除,員工平均被拖欠工資三個月以上。

報導引述美國紐約城市大學政治學教授夏明的說法,他認為,谷卓恆所爆材料真假難辨,但這個現象本身卻說明中國政局混亂,內部權力鬥爭嚴重。「習近平現在最大的問題是蛋糕越做越小,但他的胃口卻是越來越大,各派有錢優勢的人雞飛狗跳,所以會出現內部的爭鬥。現在習近平所做的,是不可能解決這些問題的」,夏明猜測,未來一兩年,類似的事件可能會越來越多。

Aggression: Clinical Features and Treatment Across the Diagnostic Spectrum addresses the role aggression plays in intermittent explosive disorder (IED), referred to as "primary aggression," and in a host of other psychiatric disorders -- from autism to posttraumatic stress. Aggression is a well-studied, multidimensional phenomenon that may be parsed by cognition, affect, and behavior. Occurring as primary aggression and as a symptom of other psychiatric disorders, aggression is both common and destructive -- of property, relationships, and the social order. The editors were instrumental in the development of the DSM classification for IED, and the book discusses the evolution of the diagnostic criteria from the 1950s to the current DSM-5. This long-range perspective informs the volume and provides a firm foundation for the clinical content, which is case-based and accessible. Moreover, each chapter is consistent in structure, moving from phenomenology to psychobiology to clinical approach and treatment. The book emphasizes that although abnormal psychobiological systems, including neurochemistry and neural circuits, may increase the risk of behavioral aggression, influences associated with genetics, epigenetics, and environmental forces are also extremely important, and these are discussed in-depth in each chapter.

Aggression and anger may play a role in the onset and maintenance of eating disorders, complicating clinical presentation and influencing prognosis and treatment. The chapter on eating disorders addresses aggression, both self- and other-directed, and provides measures with sound psychometric properties for assessing eating disorder psychopathology and aggression.

The chapter on anxiety disorders highlights the prevalence of aggression, hostility, irritability, and anger in patients with these disorders, noting that these individuals may be at increased risk due to genetic sensitivity to stress. The role of aggression in social anxiety disorder, panic disorder, and generalized anxiety disorder is explored in-depth, and current treatment recommendations are discussed.
