Semisynthetic and synthetic opioids
https://en.wikipedia.org/wiki/Opioid#Semisynthetic_and_synthetic_opioids
Synthetic opioids were invented, and biological mechanisms for their actions discovered, in the 20th century.[167] Scientists have searched for non-addictive forms of opioids, but have created stronger ones instead. In England Charles Romley Alder Wright developed hundreds of opiate compounds in his search for a nonaddictive opium derivative. In 1874 he became the first person to synthesize diamorphine (heroin), using a process called acetylation which involvedboiling morphine with acetic anhydride for several hours.[179]
Heroin received little attention until it was independently synthesized by Felix Hoffmann (1868–1946), working for Heinrich Dreser (1860–1924) at Bayer Laboratories.[193] Dreser brought the new drug to market as an analgesic and a cough treatment for tuberculosis, bronchitis, and asthma in 1898. Bayer ceased production in 1913, after heroin's addictive potential was recognized.[179][194][195]
Several semi-synthetic opioids were developed in Germany in the 1910s. The first, oxymorphone, was synthesized from thebaine, an opioid alkaloid in opium poppies, in 1914.[196] Next, Martin Freund and Edmund Speyer developed oxycodone, also from thebaine, at the University of Frankfurt in 1916.[197] In 1920, hydrocodone was prepared by Carl Mannich and Helene Löwenheim, deriving it from codeine. In 1924, hydromorphone was synthesized by adding hydrogen to morphine. Etorphine was synthesized in 1960, from the oripavine in opium poppy straw. Buprenorphine was discovered in 1972.[196]
The first fully synthetic opioid was meperidine (later demerol), found serendipitously by German chemist Otto Eisleb (or Eislib) at IG Farben in 1932.[196] Meperidine was the first opiate to have a structure unrelated to morphine, but with opiate-like properties.[167] Its analgesis effects were discovered by Otto Schaumann in 1939.[196] Gustav Ehrhart and Max Bockmühl, also at IG Farben, built on the work of Eisleb and Schaumann. They developed "Hoechst 10820" (later methadone) around 1937.[198] In 1959 the Belgian physician Paul Janssen developed fentanyl, a synthetic drug with 30 to 50 times the potency of heroin.[179][199] Nearly 150 synthetic opioids are now known.[196]

Drugs Unlimited: The Web Revolution That's Changing How the World Gets High (Mike Power, Thomas Dunne Books, 2014)
The very first thing ever bought or sold on the Internet was marijuana, when Stanford and MIT students used ARPANET to cut a deal in the early '70s. Today, you can order any conceivable pill or powder with the click of a mouse. In Drugs Unlimited, Mike Power tells the tale of drugs in the Internet Age, in which users have outmaneuvered law enforcement, breached international borders, and created a massive worldwide black market.
But the online market in narcotics isn't just changing the way drugs are bought and sold; it's changing the nature of drugs themselves. Enterprising dealers are using the Web to engage highly skilled foreign chemists to tweak the chemical structures of banned drugs―just enough to create a similar effect and just enough to render them legal in most parts of the world. Drugs are marketed as "not for human consumption," but everyone knows exactly how they're going to be used―what they can't know is whether their use might prove fatal.
From dancefloors to the offices of apathetic government officials, via social networking sites and underground labs, Power explores this agile, international, virtual subculture that will always be one step ahead of the law. (amazon) (accessible via scribd)

Abstract
Historically, drugs of abuse have come from two sources: plant products and diverted pharmaceuticals. Today, new, totally synthetic drugs produced by clandestine laboratories have become an increasingly important source of abused substances. Of particular concern are the fentanyls, a family of very potent narcotic analgesics, which first appeared on the streets in California in 1979 under the name "China White". At least 10 different analogs have been identified to date and are thought to be responsible for over 100 overdose deaths. The fentanyls are not used by any particular ethic or age group, but rather by the general heroin using population. Their use, however, does seem to be restricted to suburban, rather than urban areas, and almost exclusively to the state of California. The most potent analogs, the 3-methyl- and beta-hydroxy-fentanyls, may be up to 1000 times as potent as heroin, but are not chemically related to the opiates and therefore not detected by conventional narcotic screening tests. However, using a sensitive radioimmunoassay highly specific for the fentanyls they can be measured at the very low concentrations observed in body fluids, generally less than 10 ng/mL. It is likely that, as efforts to restrict the importation of natural products and prevent diversion of pharmaceuticals become more effective, the fentanyls and other synthetics will become increasingly important drugs of abuse.

In the mid-1980s, it wasn’t clear whether these types of “synthetic heroin” would become a plague or simply fade away. A University of California at Davis pharmacology professor named Gary Henderson studied the chemical impurities in China White and concluded that a single chemist was responsible for all of it. “Most likely he made a few grams of the drug—millions of doses—and then shut up his shop,” he told journalist Jack Shafer in 1985.
Henderson became the scientist doctors turned to when their overdose patients had strange blood samples, and the DEA consulted when it turned up inscrutable chemicals. He had already been researching fentanyl for years; his lab focused on how it was used to dope racehorses, whose urine turned up traces of it. He worked to develop a technique to identify fentanyl and began to understand the nature of the drug, including its potential for chemical manipulation. “Perhaps hundreds,” he said, when asked how many fentanyl analogues would be possible. “Maybe thousands.”
Henderson was way ahead of his time when it came to predicting the horrors not just of fentanyl but of NPS generally. “It seems we are still watching reruns of The French Connection while there is someone out there using a computer to search the chemical literature looking for new drugs to synthesize,” he told the US Senate’s Budget Committee in July 1985, a statement that was remarkably prescient. He coined the phrase designer drugs, defining them as “substances where the psychoactive properties of a drug are retained, but the molecular structure has been altered to avoid prosecution.” Often synthesized from common chemicals, they were skillfully marketed under attractive, exotic names, he added. His 1988 paper “Designer Drugs: Past History and Future Prospects” is nothing less than prophecy, speculating accurately not just on the future of NPS chemistry but on the implications for law enforcement. “In the view of this author,” he wrote, “it is likely that the future drugs of abuse will be synthetics rather than plant products. A single gram of any very potent drug could be synthesized at one location, transported to distribution sites worldwide, and then formulated (cut) into many thousand, perhaps a million, doses…. Preventing the distribution of such small amounts of the pure drug will be exceedingly difficult…. In fact, any success we may have in curtailing the distribution of natural products such as opium, coca, and marijuana and preventing the diversion of pharmaceuticals will only stimulate the development of potent synthetic substitutes.”

a view from somewhere
Therefore, hermeneuticists conclude that when researchers claim to bracket off their prejudgements, they only succeed in disguising them. For this reason, critics of scientism have called it “a view from nowhere” — meaning that its values and commitments are nowhere to be seen. Our only hope, hermeneuticists such as Heidegger argued, is to develop ways of thinking about human being that understand that people are on-going events, not static things that can be known, controlled, and repaired through the use of technicist, instrumental methods. Without such understanding, we are continually involved in a self-deception about the content and limits of human knowledge. Historically, some of our most damaging social problems can be traced to beliefs and actions justified by the warrant of objectivity. (pp. 211-13)

Recent Changes in Drug Abuse Scenario The Novel Psychoactive Substances (NPS) Phenomenon (ed. Fabrizio Schifano, Mdpi AG, 2019-1-15)
In parallel with a decrease/stabilization in the use of internationally controlled drugs, the market of novel psychoactive substances (NPS) continues increasing, year after year, with the Internet playing a pivotal role in contributing to this complex scenario. At times, although misleading, the terms 'legal highs' or 'research chemicals' have been used to describe these molecules. Overall, about 5% of 19-24-year-old European people have already experimented with them, because both NPS' intense psychoactive effects and their virtual non-detectability in routine drug screenings. Although 'novel' typically refers to molecules that have recently become a reason of current/potential public health concern, at times, the NPS scenario may indeed focus on pharmaceutical molecules. These may have either not entered the market, or are currently prescribed, with their potential for misuse resulting from the relating high dosages/idiosyncratic ways of self-administration. A concurrent use of a range of different NPS, and/or medications, is frequently being reported and this may be a reason of further clinical complications. Given their complex pharmacodynamics, there are increasing levels of concern about the onset of acute/chronic psychopathological associated with NPS intake. This Special Issue aims at providing an overview of a range of NPS-related issues, e.g., preclinical, epidemiological, and clinical pharmacological; the medical and psychopathological consequences associated with their intake; and, finally, the analytical chemistry and forensic analysis challenges associated with the NPS phenomenon. What you need to know
- Novel psychoactive substances (NPS, “legal highs”) are compounds designed to mimic existing established recreational drugs. They can be grouped into four main categories: stimulants, cannabinoids, hallucinogens, and depressants
- Legislation regarding NPS varies internationally. In the UK it is now illegal to distribute or sell NPS, but possession is not a criminal offence
- NPS should not be regarded as safer than established recreational drugs
- The most commonly clinically encountered NPS are stimulants (such as mephedrone) and cannabinoids (such as “spice”)
- Psychiatric and rehabilitation units, prisons, and schools face particular challenges in detecting and preventing use
In 2016 the Psychoactive Substances Bill banned trading but not possession of all current and future novel psychoactive substances (NPS), sometimes incorrectly called “legal highs,” in an attempt to overcome rapid proliferation of these compounds. Over 560 substances are currently monitored by the European Monitoring Centre for Drugs and Drug Addiction, with 100 new agents identified in 2015 alone. Stimulants and synthetic cannabinoids account for the vast majority and are the types most commonly clinically encountered.1 Online purchases are increasing according to the 2016 Global Drug Survey,2 potentially in response to legislative changes, as is overall NPS use: lifetime consumption was reported by 8% of younger individuals in 2015, up from 5% in 2011, with figures relatively similar between sexes and different countries.3

Drug Dealer, MD: How Doctors Were Duped, Patients Got Hooked, and Why It’s So Hard to Stop (Anna Lembke, Johns Hopkins University Press, 2016)
Three out of four people addicted to heroin probably started on a prescription opioid, according to the director of the Centers for Disease Control and Prevention. In the United States alone, 16,000 people die each year as a result of prescription opioid overdose. But perhaps the most frightening aspect of the prescription drug epidemic is that it’s built on well-meaning doctors treating patients with real problems.
In Drug Dealer, MD, Dr. Anna Lembke uncovers the unseen forces driving opioid addiction nationwide. Combining case studies from her own practice with vital statistics drawn from public policy, cultural anthropology, and neuroscience, she explores the complex relationship between doctors and patients, the science of addiction, and the barriers to successfully addressing drug dependence and addiction. Even when addiction is recognized by doctors and their patients, she argues, many doctors don’t know how to treat it, connections to treatment are lacking, and insurance companies won’t pay for rehab.
Full of extensive interviews―with health care providers, pharmacists, social workers, hospital administrators, insurance company executives, journalists, economists, advocates, and patients and their families―Drug Dealer, MD, is for anyone whose life has been touched in some way by addiction to prescription drugs. Dr. Lembke gives voice to the millions of Americans struggling with prescription drugs while singling out the real culprits behind the rise in opioid addiction: cultural narratives that promote pills as quick fixes, pharmaceutical corporations in cahoots with organized medicine, and a new medical bureaucracy focused on the bottom line that favors pills, procedures, and patient satisfaction over wellness. Dr. Lembke concludes that the prescription drug epidemic is a symptom of a faltering health care system, the solution for which lies in rethinking how health care is delivered. (amazon)
The Chinese Heroin Trade: Cross-Border Drug Trafficking in Southeast Asia and Beyond (Ko-lin Chin, Sheldon X. Zhang, NYU Press, 2015)
In a country long associated with the trade in opiates, the Chinese government has for decades applied extreme measures to curtail the spread of illicit drugs, only to find that the problem has worsened. Burma is blamed as the major producer of illicit drugs and conduit for the entry of drugs into China. Which organizations are behind the heroin trade? What problems and prospects of drug control in the so-called “Golden Triangle” drug-trafficking region are faced by Chinese and Southeast Asian authorities?
In The Chinese Heroin Trade, noted criminologists Ko-Lin Chin and Sheldon Zhangexamine the social organization of the trafficking of heroin from the Golden Triangle to China and the wholesale and retail distribution of the drug in China. Based on face-to-face interviews with hundreds of incarcerated drug traffickers, street-level drug dealers, users, and authorities, paired with extensive fieldwork in the border areas of Burma and China and several major urban centers in China and Southeast Asia, this volume reveals how the drug trade has evolved in the Golden Triangle since the late 1980s. Chin and Zhang also explore the marked characteristics of heroin traffickers; the relationship between drug use and sales in China; and how China compares to other international drug markets. The Chinese Heroin Trade is a fascinating, nuanced account of the world of high-risk drug trafficking in a tightly-controlled society. (amazon)

Disillusioned with communism, he returned to the US in 1980 with his wife and their four children.
There, he founded Rittenberg Associates, a consulting company that helped businesses from Colgate Palmolive to Warner Music to Intel, Microsoft and PricewaterhouseCoopers to establish themselves in China. (i.e. even revolutionaries need to do businesses, which, of course, are revolutionary businesses)
“I think China has to face the fact that Mao was one of the worst people in human history,” was Rittenberg’s assessment of Mao in old age. “He was a genius, but his genius got completely out of control, so he was a great historic leader and a great historic criminal. He gave himself the right to conduct social experiments that involved upturning the lives of hundreds of millions of people, when he didn’t know what the outcome might be. And that created famines in which tens of millions died, and a revolution in which nobody knows how many died.” Rittenberg explained that his idealism and the belief that he was taking part in the development of a new and better world blinded him to the atrocious persecution and murder of even close friends of his. “It’s a kind of corruption, exactly the kind of corruption that ruins the whole thing,” he said. “I believed I was part of history.

failed artist (artiste manque)
Using an idea from Schopenhauer, Rank develops a metaphor: Life is
a loan, death the repayment. The artist type takes the loan and spends or invests
it—willingly. Such a person accepts the finite term of the loan, and makes choices in accordance with that reality. The neurotic,
in contrast, cannot willingly accept the loan with its limit. He or she
vacillates, paralyzed by anxiety and doubt, refusing to commit the
life-loan. At repayment time, the neurotic hopes—pathetically—to flout the
limit. “I haven’t begun yet. I should not have to die—I have not
really lived!”
