Antipsychotic Polypharmacy May Reduce Long-Term Rehospitalization Risk, Study Finds
A combination of clozapine plus aripiprazole was associated with the lowest risk of psychiatric rehospitalization among patients with schizophrenia who were followed for up to 20 years in Finland, according to a
studypublished Wednesday in JAMA Psychiatry.
As many as 30% of patients with schizophrenia are treated with a combination of antipsychotics, although the practice is controversial because of its lack of evidence of tolerability, safety, and efficacy, wrote Jari Tiihonen, M.D., Ph.D., of the Karolinska Institutet in Sweden and colleagues.
In this study on the long-term use of antipsychotic polypharmacy in schizophrenia, researchers examined seven monotherapies and 22 antipsychotic combinations, excluding polypharmacy periods shorter than 90 days.
Researchers analyzed data on all Finnish patients with schizophrenia treated in an inpatient setting from 1972 to 2014, and the 62,250 patients were followed for a median of 14 years, with researchers pulling data on their dispensation of antipsychotic medication. The primary outcomes studied were the number of psychiatric rehospitalizations, with hospitalizations due to physical illnesses and deaths serving as secondary outcomes.
During that time 58% of participants were readmitted for psychiatric inpatient care.
The study findings include the following:
The researchers concluded that certain types of polypharmacy may be beneficial in the treatment of schizophrenia. “Although the results do not indicate that all types of polypharmacy are beneficial, the current treatment guidelines should modify their categorical recommendations discouraging all antipsychotic polypharmacy in the maintenance treatment of schizophrenia,” they wrote.

- The advantage of the clozapine plus aripiprazole combination was greater for patients with first-episode schizophrenia, who were 23% less likely to be rehospitalized than other patients with first-episode schizophrenia.
- Among the 29 therapies examined, patients taking quetiapine monotherapy had highest risk of psychiatric rehospitalization, but adding on any another antipsychotic resulted in a better outcome.

The rate of suicides is at its highest level in nearly 30 years. Suicide notes have long been thought to be valuable resources for understanding suicide motivation, but up to now the small sample sizes available have made an in-depth analysis difficult. Explaining Suicide: Patterns, Motivations, and What Notes Reveal represents a large-scale analysis of suicide motivation across multiple ages during the same time period. This was made possible via a unique dataset of all suicide notes collected by the coroner’s office in southwestern Ohio 2000–2009.
Based on an analysis of this dataset, the book identifies top motivations for suicide, how these differ between note writers and non-note writers, and what this can tell us about better suicide prevention. The book reveals the extent to which suicide is motivated by interpersonal violence, substance abuse, physical pain, grief, feelings of failure, and mental illness. Additionally, it discusses other risk factors, what differentiates suicide attempters from suicide completers, and lastly what might serve as protective factors toward resilience.

He is now known for a collection of quotations from and reports about Presocratic philosophers.[a] This work, entitled Die Fragmente der Vorsokratiker (The Fragments of the Pre-Socratics), is still widely used by scholars. To begin to approach the essence of language, Heidegger considers poetry, which he regards as the purest form of speaking. The poem he chooses to discuss is “A Winter Evening,” by the lurid Austrian genius Georg Trakl (1887–1914), a master of morbid imagery who died of an overdose of cocaine. It might be translated as follows:
A Winter Evening
When the snow falls at the pane,
evening bells will long resound—
the table set for many guests
in a well-provided house. Some, along their wandering,
follow dark roads to the gate.
Cooling juices from the earth
will feed the golden tree of grace.
Silent drifter steps inside,
a painful threshold turned to stone.
There, in purest brightness shine,
on the table, bread and wine.
Trakl’s poem speaks of numerous alluring things: snow, bell, window, pain, threshold, falling, and resounding. By naming these entities, the poem is not just giving titles to things that are already found present-at-hand. Instead, it calls these things before us for the first time. This calling summons the objects into a true nearness, which (we already know) is also a kind of distance.
The presence of these things in the poem is something much deeper than the presence of physical bells or snowflakes lying before us. If I were to see a snow-covered bell in my front yard, I might simply represent it as a set of visible qualities. But if a bell is named in the poem, it is summoned before me in a presence that is simultaneously an absence. In the poem, the thing things: the bell bells, to use a phrase that Heidegger himself never uses. In this way, the bell is gathered into the mirror-play of the fourfold of earth and sky, gods and mortals. The bell unites the fourfold even though the four poles remain distinct. This united fourfold is the world.
