緣起和理念 (福德身心診所)

 緣起和理念


(一) 緣起之前

2011,我離開待了二十八年的北榮,南行,來到虎頭山麓的桃榮 (台北榮總桃園分院),主任七年,多待兩年,2020 退休。

北榮是醫學中心,分工極細,每個人只要忙自己專精的部分即可。桃榮是區域教學醫院,精神科二百四十張床 (急性慢性日間),編制緊縮,只有四位主治醫師,每個人忙裡忙外什麼都要忙。

在北榮,除了一般精神醫療,我主要用心在心理治療,尤其是自體心理學。在桃榮,除了一般精神醫療,我主要用心在帶領醫療團隊,大家一起勞動。

我感念那九年,一起共事過的,護理心理職能治療社工就服諸師,我跟這群年輕人說,這裡是一個學習型組織,大家能走多遠,就走多遠。

你可以說,北榮二十八年,我其實鎖在塔裡;桃榮九年,我才行走在大地。

2020 六月退休,赴竹北,在友人吳醫師診所,待了三年半。但 2019 九月起,就與台齡和老翁 (輔大心理所翁開誠老師),一起開始,合辦「竹北息壤人文講座」,每回六講,今年將入第七回。

診所是基層醫療,業務比醫院單純,是把自己種在土裡,照顧一片地。

(二) 緣起

這件事,跟廟有關。

2022 九月,我因籍設中壢,接到通知,去光明公園旁,里民活動中心,打新冠追加疫苗。

我本不知,中壢有這麼大的一個綠意盎然的公園。

而且,民權元化路口,有一間樸素的土地公廟。

我心想,診所開在這裡,隨時可以,公園小坐,實在太好了。

民權路上,大樓店面,面積太小,不合適。

中豐路上,老街溪站,工程甫告一段落,興國停車場前,有一店面頂讓,未成。

2023 十月,幾番迤邐,遂來到中美路。

2024 一月二十八日,開幕前夕,在診所對面,裕國路小巷,找到興國里福德寺。這是一個潔淨的,與生活肌理密合的,在地的土地公廟。

意思是說,我本不知,一切皆由廟而起。

(三) 理念

精神醫療,心理治療,發生在一個乾淨明亮的地方。

我們每個人,人生際遇,有命運的因素,遭逢不順遂,只能盡人事,度過這些坎陷。

意思是說,人生必有其,相隨症狀。

但是做人要有信心,症狀不足畏。

必要時,輔諸藥物,以緩解之。但是,服藥不要喧賓奪主,「多少才夠」,非 how much is enough,是 how little is enough,意思是說,要尋找「最小有效劑量」,療程告一段落,即應討論,如何減少劑量,簡化處方。

人,有其個性,有其情緒,有其無以名之的主觀因素。意思是說,除外在客觀際遇命運,人的困難,可以是個人內在的作繭自縛。這時,心理治療,可以考慮。但是,比照上述藥物治療,「多少才夠」,非 how much is enough,而是 how little is enough。意思是說,心理治療,是不得已的,能夠交給生活歷練去學習度過的,心理治療,就不應強作解人,急著進場。而且,能夠短期治療的,就不應沒完沒了的長期治療。

簡單講,精神醫療,心理治療,最重要的是,「將生命還給生命」,幫助,促成,和祝福一個人,「擁有他自己的人生」 (to get a life)。

意思是說,精神醫療,心理治療,就是流浪貓的流水席,就是陌生人的善意,就是放牛吃草,就是明儒陳白沙講的「鳶飛魚躍,回到自然」。

然後,這一切,都發生在,種在土裡,照顧一片地的,土地公廟。

我剛剛說過,那是一個乾淨明亮的地方。

Robert Desjarlais is a cultural anthropologist and photographer who lives in New York, where he teaches at Sarah Lawrence College.  A native of Massachusetts, he has been the recipient of several distinguished awards, including a Guggenheim Fellowship. He received his PhD in anthropology from the University of  California, Los Angeles, in 1990, and was a NIMH post-doctoral research fellow at Harvard University from 1990 to 1992. He was recently the Alice Ilchman Chair in International and Comparative Studies at Sarah Lawrence.


He has conducted anthropological research in several distinct settings, ranging from a Tibetan Buddhist people in the Nepal Himalayas to a shelter for the homeless in downtown Boston. His main scholarly interests relate to the fields of cultural, psychological, and medical anthropology, and phenomenological approaches in anthropology. He is the author of four books, the most recent of which is entitled Counterplay: An Anthropologist at the Chessboard, published by the University of California Press. He is currently completing a new book titled Subject to Death: Life and Loss in a Buddhist World.

Andre Gide once said that Feodor Dostoevsky "lost himself in the characters of his books, and, for this reason, it is in them that he can be found again." In "Dostoevsky: The Author as Psychoanalyst", Louis Breger approaches Dostoevsky psychoanalytically, not as a "patient" to be analyzed, but as a fellow psychoanalyst, someone whose life and fiction are intertwined in the process of literary self-exploration.Raskolnikov's dream of the suffering horse in "Crime and Punishment" has become one of the best known in all literature, its rich imagery expressing meaning on many levels. Using this as a starting point, Breger goes on to offer a detailed analysis of the novel, situating it at the pivotal point in Dostoevsky's life between the death of his first wife and his second marriage. Using insights from his psychological training, Breger also explores other works by Dostoevsky, among them his early novel, "The Double", which Breger relates to the nervous breakdown that Dostoevsky suffered in his twenties, as well as "Notes from Underground", "The Possessed", "The Idiot", "The Brothers Karamazov", and so forth. Additionally, details from Dostoevsky's own life - his compulsive gambling, his epilepsy, his philosophical, political, religious, and mystical beliefs, and the interpretations of them found in existing biographies - are analyzed in detail.

It is sometimes proclaimed that crises generate creative powers. An idea to consider, beyond the banal advertising or entrepreneurial statements about the fruitful nature of crises (political, social, economic, or personal). It is the psychic, literary, and philosophical aspect of the notion of crisis that is explored here in its relationship to creation. The crisis of creativity: silence, withdrawal, sterility. Everyone knows these periods of emptiness, of depressive obstruction. Is the creativity of the crisis the simple reversal of it?

As Deleuze or Beckett, Nietzsche or Foucault knew, but also many modern artists and creators, it is not easy to endure the instability required by all creation, the forces of bewilderment that it unleashes, everything like its undeniable ecstasy. Creation is undoubtedly an apprenticeship in insecurity.

上述說明,尼采錯過的,是現代性的災難現場,

有趣的是,尼采預見的,是後現代後結構的廢墟荒原,

另,為什麼俄國,尤其蘇維埃之前的那個俄國,這麼虛無?我想,跟天寒地凍,喝了太多伏特加有關,我倒是沒有聽說,俄國有酒神,或酒神住在俄國,意思是說,酒神來自地中海沿岸,生性怕凍,俄國人的醉,遂只是酒鬼,純粹的醉,意思是說,俄國的虛無,是 (讓人)宿醉頭痛的虛無,這種虛無,殖民中國百年,就是共匪的虛無,包子大醉連環炮後曾說,我們這些紅色基因屠殺人民的共匪,不是死在酒桌,就是床上,意思是說,趙家人不是醉死,就是精盡縮陽,馬上風而亡,你要理解,前者需看戒酒門診,後者需韋小寶的姐姐,京城名妓韋春花來搭救,

另,虛無主義是症狀,所有耐不住症狀的人,都急著索討解藥,殊不知解藥也是症狀的一部分,意思是說,精神分析,心理治療,也是症狀,意思是說,尼采的哲學,奔著克服 (overcome) 虛無而去,但是,西谷啟治說,這個克服,是自我克服 (self-overcoming),意思是說,你要愛你的命運,你要讓虛無穿過你,再看你還能,剩下什麼殘骸,西谷此說,就是尼采留給我們,最重要的一課,

Nihilism is a highly negative system of thought with roots in early Greek philosophy. It came into prominence as a major movement with Friedrich Nietzsche's unparalleled assault on Christianity and Christian morality. It became a dominant theme in the dark philosophical system known as existentialism, and thus became an important force in nineteenth century literature and in twentieth century ideologies. It seeks destruction of one or more aspects of society without offering a viable alternative, frequently assuming that the better world will automatically appear after the old world is obliterated. Loathing the building blocks of the present system, nihilism asserts that the better world will be composed of new, but unspecified, components. French philosophy during and after the French Revolution, and virtually all nineteenth century Russian literature, was dominated by nihilism. German Nazism had a nihilistic base which was carefully concealed by racist rantings. Marxism, with so many of its ideas stolen from Russian and French nihilists, proclaimed that faulty economics brought about misery and poverty which would be eradicated by the new but unspecified and undescribed socialist ethic. Revolutionary systems in the twentieth century have delved heavily into the rich trove of nihilist literature to promote, describe, and espouse revolutions which have marked much of that century. Few contemporary nihilists have offered any new insights into reality, choosing only to manipulate the basic concepts heretofore advanced. But the earlier nihilistic ideas have become an all-inspiring training primer for nihilists of future polities. To understand the philosophy of nihilism is to understand the revolutions that have continued to challenge modern societies.

Dualism,兩分法,splitting,「一 vs 二」的世界,實屬學習初級階段,人卻以為,能夠拆解世界,將之一分為二,就是高級思考,

Non dualism,非兩分法,進入「大於等於三」的世界,這個世界,友,敵,非友非敵 (ie. 陌生人),先友後敵,先敵後友,敵友非恆態,敵友難分,意思是說,兩分法,是一個井井有條 (orderly),沒有背叛,沒有兩難,不需懺悔,不需寬恕的世界,非兩分法,則是一個,處處都模稜兩可 (ambiguous),讓人捨不得,又放不下的世界,

小結,兩分法,是一個,簡單粗暴,但是清晰完美的分類,非兩分法,是一個,糨糊邏輯,明顯腦筋不清楚的分類,前者,就是邊緣性人格構造 (BPO,borderline personality organization) 的思惟,就是文革挑起無盡互鬥的機制,後者,就是風塵僕僕的中年人,微笑看待,大千世界,云云眾生,和自己窩囊的一生,

自由 (negativity vs vitality)

Weak Thought (Weak Ontology)

A label applied to the work of Italian philosopher Gianni Vattimo. Although Vattimo adopted this description of his thought, his own term for what he is trying to do in his work is in fact ‘weak ontology’. He derives the idea of a weak ontology from his reading of Martin Heidegger and Heidegger's student Hans-Georg Gadamer, whose key work he translated into Italian. On Vattimo's reading of Heidegger there is a profound separation between language and reality; language on this view can at best convey an interpretation of reality, but never the thing itself. Hence it is a ‘weak’ not ‘strong’ ontology. But in contrast to a number of other philosophers in the postmodern anti-foundation tradition, Vattimo does not interpret this state of being as a crisis for metaphysics.
